Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us! New patient appointments are for an hour. 

This is not the scheduling portal. This is to make a request that we will attempt to work out with you. The scheduling portal is: www.appointments.mychirotouch.com. The scheduling portal is only available if you have emailed us a request to join, and have signed up.
